Description

What is Mounjaro?

Mounjaro is a once-weekly injectable medication that contains tirzepatide, a dual GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) receptor agonist. Initially used for the treatment of type 2 diabetes in adults, it has now been licensed for weight loss and weight managemnt for adults aged 18 and over.

In clinical trials, tirzepatide demonstrated significant results in reducing body weight, as it works by slowing gastric emptying, promoting a feeling of fullness, and reducing appetite. This makes it a promising option for those with obesity or individuals who require medical assistance to achieve and maintain a healthy weight.

How does Mounjaro Work?

Mounjaro works by mimicking the effects of two naturally occurring hormones in the body, GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). These hormones play essential roles in regulating appetite, metabolism, and blood sugar, making Mounjaro particularly effective for both blood sugar control and weight management. Here’s a closer look at how it works:

1. Activation of GLP-1 Receptors

Mounjaro stimulates the GLP-1 receptors in the body, which triggers several effects that help with weight loss and blood sugar management. By activating these receptors, Mounjaro:

  • Reduces appetite, helping users feel less inclined to overeat.
  • Slows down digestion, which prolongs the feeling of fullness after meals. This delay in gastric emptying helps prevent sharp spikes in blood sugar, providing a smoother release of glucose into the bloodstream.\

2. Activation of GIP Receptors

In addition to its GLP-1 effects, Mounjaro activates the GIP receptors. The activation of these receptors works synergistically with GLP-1 to enhance the medication’s impact on appetite and metabolism, as well as improve insulin sensitivity. Specifically, GIP activation:

  • Further enhances the feeling of satiety, helping users feel fuller for longer periods.
  • May improve the body’s response to insulin, leading to more efficient glucose metabolism.

When combined with a reduced-calorie diet and regular physical activity, Mounjaro can lead to meaningful weight loss and contribute to better overall metabolic health.

Warnings

Overconsumption of fat and calories can reduce the weight-loss effect of Mounjaro. As with any weight-control program, lifestyle changes such as a balanced diet and physical activity are key to achieving sustainable results.

Mounjaro is only available for patients who have a BMI of 30 kg/m² or more. Or a BMI of at least 27 kg/m² and weight-related health problems.

Mounjaro may interact with certain medications, as it slows stomach emptying, which can alter the effect of medicines that need to pass through the stomach quickly. It is important to consult your prescriber to adjust the timing or dosage of other medications as needed.

Long-term use is required for effective weight management with Mounjaro. It is not a quick fix for obesity. Discontinuing Mounjaro may result in regaining much of the weight you lost during treatment. It should be taken as part of a comprehensive weight management plan.

Serious Health Risks and Warnings

  • Pancreatitis: Mounjaro may increase the risk of pancreatitis (inflammation of the pancreas). Seek immediate medical attention if you experience severe pain in the upper part of your abdomen that may radiate to your back, along with nausea, vomiting, fever, or a fast heartbeat.

  • Severe Allergic Reactions (Anaphylaxis): Although rare, Mounjaro can cause a severe allergic reaction. Seek emergency help if you experience symptoms like raised, red, itchy rash (hives), severe dizziness, difficulty breathing, swelling of the face, lips, or tongue, or loss of consciousness.

Contraindications and Special Considerations

Mounjaro should not be used by the following individuals or in certain conditions:

  • Patients under 18 years of age or over 85 years of age
  • Individuals with obesity due to endocrinological or eating disorders, or those using medications that may cause weight gain
  • Patients with severe renal (kidney) or hepatic (liver) impairment
  • Those with a sustained increase in resting heart rate
  • Patients with type 2 diabetes, particularly those with diabetic gastroparesis (delayed stomach emptying) or ketoacidosis
  • Individuals with inflammatory bowel disease (IBD)
  • Patients with pancreatitis or a history of pancreatitis
  • Those with current or past gallbladder disease (cholelithiasis or cholecystitis)
  • Individuals with a history of medullary thyroid cancer (MTC) or other thyroid disorders
  • Pregnant or breastfeeding women
  • Patients with a known hypersensitivity to tirzepatide or any of its ingredients

Additional Precautions

  • Kidney Problems: Mounjaro may affect kidney function, particularly if dehydration occurs due to side effects like nausea, vomiting, or diarrhea. Be mindful of your fluid intake to avoid dehydration.

  • Gastrointestinal Side Effects: Common side effects of Mounjaro, such as nausea, vomiting, diarrhea, and constipation, can impact your overall hydration and nutrition. If these side effects persist or become severe, consult your healthcare provider.

  • Monitoring: Regular monitoring of your health, especially for thyroid or pancreas-related conditions, is essential while on Mounjaro.

Directions

Mounjaro is available in multiple strengths to accommodate the needs of different patients. The typical treatment journey starts with a low dose and gradually increases over time in a process called dose titration. This allows your body to adjust to the medication and helps you reach the most effective dose for your weight loss goals. The dosage schedule may vary depending on how well you tolerate the treatment and how satisfied you are with your progress.

Month Dosage (per week)
1 2.5mg (Starting dose)
2 5mg (Maintenance dose)
3 7.5mg
4 10mg (Maintenance dose)
5 12.5mg
15mg (Maintenance dose)

Important Notes:

  • 2.5mg is always the starting dose for all patients, and this will be gradually increased as needed.
  • Dose adjustments should be made based on how you feel and how well you are tolerating the medication. There is no rush to increase the dose, and you should progress to a higher dose only if you are comfortable and ready.
  • If you are feeling well and experiencing the desired weight loss at your current dose, you can remain on that dose as long as needed. Most people tend to remain on one of the maintenance doses (5mg, 10mg, or 15mg) for the long term.
  • If you experience side effects, you may consider staying at your current dose or lowering it. Always consult with your healthcare provider for guidance.
  • The decision to move up to a higher dose should only be made if you are tolerating the current dose well and would like to increase your rate of weight loss.

FAQs

How Effective is Mounjaro?

In studies where people used Mounjaro for over a year (72 weeks), different doses were tested, and the results showed the following average weight loss:

  • Low Dose (5 mg): People lost about 16.1 kg (2.5 stone) on average, which is roughly 15% of their starting weight.
  • Medium Dose (10 mg): Those taking 10 mg weekly lost an average of 22.2 kg (3.5 stone).
  • High Dose (15 mg): On the highest dose, participants lost around 23.6 kg (3.7 stone), about 21% of their starting weight.
